  3. First and last name, as it appears on the Enerjet (EG) or tour operator itinerary, of the individual(s) for whom the advanced seat selection, or special needs seat assignment request, is for
  4. American Express, MasterCard or Visa credit card (a credit card is not required for special needs seat assignment requests)
  5. A first, second and third preferred seating choice
